We are currently accepting new patients at Tadworth Medical Centre who live within our catchment area.  Our catchment area is Tadworth, Epsom Downs, Langley Vale, Kingswood, Lower Kingswood, Burgh Heath, Tattenham Corner, Walton-on-the-Hill, Nork and part of Banstead.

How to register…
If you would like to join the practice please complete a registration form which can be downloaded below or can be picked up from reception.  Please bring the completed form along to reception with your NHS medical card if you have it, as well as forms of ID for each person wishing to register, one of which must confirm your home address. The information on the medical card will speed up the process of getting your records from your previous practice.  An NHS medical card is issued when you first register with a GP. It contains your NHS number and other information such as your name, address, date of birth, and details of your registered practice or individual doctor.

When you register, we will ask you to make an appointment to see the practice nurse for a health check as well as complete a new patient questionnaire (this can also be downloaded below).  This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.

What happens next…

When you have completed and returned the form(s), your medical records will be transferred from your previous doctor’s surgery to Tadworth Medical Centre. Your new medical card will then be posted to your home address.  You do not need to inform your existing practice that you are moving, although it may be courteous to do so.   

Informing us of a change of details…
If you change your name or address could you please remember that it is essential to inform the Practice of your new details, telephone number and postcode in writing.
